Erroneous Diagnosis Report?

Well to make a long story short I have been suffering with migraine headaches nearly everyday now for 4 months consecutively and I am being seen by a neurologist and an ENT. ( to no relief) The neurologist has prescribed a slew of drugs that mask my pain (and I refuse to take them as they interrupt my life more than the actual headaches) so FINALLY as my patience began to wear thin with my neurologist and his unprofessional-ism e.g. : not listening to my worsening symptoms as he reads the chart, not going over test results, 5 minute consultation, and taking personal cell phone calls while he’s supposed to be hearing me out, handing me prescriptions as he’s talking on cell while escorting me out with hand gestures, and the MOTHER LOAD is this: when I had enough I thought to ask receptionist for copy of dignosis to look over myself because doc was obviously to busy she complied and handed me a copy. As i skimmed the report I found MAJOR fallacies within the diagnosis report! Things like I had diabetes, (I don’t) that I had lumbosacral nerve damage, (never checked for whatever that is) that I had such and such BP (never had vitals done there) that I had little mobility in my feet and ankles (but I don’t, and my reflexes were NEVER checked) and that I had a sinus infection due to mucopurulent drain (which I never had) the list goes on and on! He even put I had constipation due to meds, which I NEVER CLAIMED due to I never took the meds longer than one day! When he was nowhere to be found I asked the receptionist what could be done about this and she "quickly" shushed me to a room and said "hold on I will get you the other doctor" (which by the way happens to be HIS physician assistant DAUGHTER) she tried to un-do the "damage" prescribed me more meds then requested more tests. My question is this:
1. Has this ever happened to you?
2. Is this normal?
3. Would you feel safe in these hands?
